    The BBL Procedure: What Happens on Surgery Day

    Step 1: Liposuction (Fat Harvesting)

    Under general anaesthesia, I begin with liposuction. Using tumescent fluid and a power-assisted cannula, fat is carefully extracted from the abdomen, flanks, lower back, and sometimes the inner thighs. The goal isn’t just to collect fat — it’s to sculpt these donor areas so the overall body contour improves. Think of liposuction as half the result.

    Step 2: Fat Processing

    The harvested fat is purified to remove blood, oil, and damaged cells. I use a closed-system decanting method that preserves the highest percentage of viable fat cells. The quality of processing directly affects how much fat survives long-term. Expect about 60-70% of transferred fat to survive permanently; the rest is absorbed by the body over the first 3 months.

    Step 3: Fat Injection (Grafting)

    The purified fat is injected into the buttocks using multiple small deposits across different layers of the subcutaneous tissue. This micro-droplet technique maximises fat survival by ensuring each fat cell is close to a blood supply. I shape the buttocks in three dimensions — projection (how far it sticks out), width (hip curves), and upper pole fullness (the “shelf” look many patients want).

    BBL Recovery: What to Expect

    The First 2 Weeks

    You cannot sit directly on your buttocks for 2 weeks. This is the single most important recovery rule. Sitting compresses the newly transferred fat cells before they’ve established a blood supply, killing them. You’ll sit on a BBL pillow (which shifts pressure to your thighs) or lie on your stomach/side.

    Swelling will be significant. Your buttocks will look larger than the final result — don’t panic, and don’t celebrate prematurely. About 30-40% of that volume is swelling that will resolve. The liposuction areas will be bruised and tender. Compression garments are worn 24/7.

    Weeks 3-6

    You can begin sitting normally again around week 3, though I recommend continued use of the BBL pillow through week 6 when possible. Light walking is encouraged from day one, but no gym, no running, no squats. You can return to desk work at week 2-3 with a BBL pillow. Swelling decreases noticeably during this phase.

    Months 2-3

    Exercise gradually resumes. Start with walking and light cardio, then progress to lower-body strength training around week 10. Gluteal exercises (squats, hip thrusts) actually help maintain your BBL results long-term by building the muscle underneath the fat graft.

    Months 3-6: Final Results

    By month 3, the surviving fat cells are permanent. Your final shape is essentially set. Some subtle changes continue through month 6 as residual swelling resolves. Your BBL results will last for decades as long as you maintain a stable weight. Significant weight loss will shrink the transferred fat, and significant weight gain will enlarge it — proportionally with the rest of your body.

    How much fat do I need for a BBL?

    I typically harvest between 2 and 4 litres of fat through liposuction, of which 800ml to 1,500ml (per side) is injected into the buttocks after processing. You don’t need to be overweight — even women at a healthy BMI of 23-25 usually have enough donor fat for a good result. During your consultation, I assess your donor sites and give you a realistic projection.

    When can I sit down normally after a BBL?

    Two weeks is the strict minimum. During those first 14 days, use a BBL pillow any time you must sit (it shifts weight to your thighs and off the buttocks). From weeks 3-6, you can sit normally but I still recommend the pillow for extended sitting. After 6 weeks, sit however you like. The transferred fat is well-established by then.

    Will I have scars from a BBL?

    The liposuction incisions are tiny — about 3-4mm each — and placed in hidden locations (inside the belly button, along the bikini line, in natural skin creases). The injection sites on the buttocks are even smaller. By 6-12 months, these scars are virtually invisible in most patients. You will not have any large or obvious scars.

    Can I combine a BBL with other procedures?

    Yes. The most common combination is BBL with a tummy tuck — sometimes called a “snatched waist” package. I also combine BBL with breast augmentation or a breast lift for patients who want a full-body transformation. Combined procedures are safe when operating time stays within safe limits and the patient is healthy enough for extended anaesthesia.

    How to Maximise Your BBL Results

    Surgery is half the equation. What you do before and after determines whether you get good results or exceptional ones. Here’s my advice based on hundreds of BBL procedures:

    Before Surgery

    • Reach a stable weight 2-3 months before surgery. Crash dieting right before your procedure reduces available donor fat and compromises healing. If you want to lose weight, do it well in advance.
    • Stop smoking at least 6 weeks before. Nicotine constricts blood vessels and dramatically reduces fat graft survival. This includes vaping and nicotine patches.
    • Avoid blood-thinning supplements. No aspirin, ibuprofen, fish oil, vitamin E, or green tea extract for 2 weeks before surgery. These increase bleeding during liposuction.
    • Eat a protein-rich diet. Your body needs protein to heal. Start increasing your intake 2 weeks before surgery — lean meats, eggs, fish, legumes.

    After Surgery

    • Stay off your buttocks. I cannot stress this enough. Two weeks minimum on your stomach or side. Use the BBL pillow religiously when you must sit.
    • Wear your compression garment. It controls swelling, supports the liposuction areas, and helps your skin conform to its new contour. Skipping it leads to worse results.
    • Walk daily. Light walking from day one promotes circulation, prevents blood clots, and speeds recovery. Short, slow walks — not hikes.
    • Maintain your weight. Gaining weight after a BBL will enlarge your buttocks (and everywhere else). Losing weight will shrink them. Stability is the goal.
    • Sleep on your stomach or side for 6 weeks. A body pillow makes this more comfortable. Many patients say they got used to stomach sleeping quickly.
    • Start gluteal exercises at month 3. Squats, hip thrusts, and lunges build the gluteal muscles beneath your fat graft, adding additional projection and maintaining long-term shape.
